Ritual:

Ritual:
The Diviner overlays the Sitemap of Care atop the Incident Form and traces the broken handoffs with a silver pen.
Each break is assigned a lineage: human lapse, system timeout, or ritual omission.
Sigils for each lineage are affixed to the margin and ranked by recurrence.

Outcome:
A prioritized root cause list written in antiseptic ink and sealed with a clipped staple.

Purge — Improve

Name: Sterilize and Suture
Officiant: Surgical Mender and Process Nurse
Artifact: Indexing Needle and Rollback Suture

Ritual:
The Mender threads the Indexing Needle through the chart metadata while the Process Nurse tightens the Rollback Suture around the offending workflow.
Corrections are offered as atonements: one corrected checklist for each trimmed shortcut.

Mantra:

We do not erase memory.
We reassign duty.

Ritual:
The Audit Sister reads the revised control plan into the Control Ledger while the Control Choir repeats the acceptance metrics three times.
The tamper ribbon is tied and the audit token is threaded onto the Consent Band Tally.

Control Oath:

  • Monitor: the Triage Monk will murmur the key metric at each shift change.
  • Escalate: the Compliance Chaplain will toll the bell on threshold breach.
  • Remember: the Chart Diviner will file a memorial note for every suture placed.
