      Placed next to sections where recurring issues have been linguistically harmonized but not structurally resolved.

      Displayed over time blocks with no recorded incidents and no declared downtime, regardless of reported strain.

    • Metrics of Care Overlays
      Used to mark intervals where some acknowledgment of care was logged, even if no change in schedule or load occurred.

    Some teams apply LC-04 purely by template: if the report renders, the seal prints.
