Purpose

This registry lists confirmed instances where witness testimony arrived at a layer that could not initiate action. It does not assess whether action was warranted. It records only that the gap existed and was archived.

Entries are additive. Resolution is not required for inclusion. Presence in this registry does not imply negligence at any layer.


Registry Format

Each entry carries:

  • Incident identifier
  • Receiving layer (what held the testimony)
  • Authority layer (what could have acted)
  • Gap type (structural / routing table / unknown)
  • Citation survival (whether the testimony was later cited beyond its class)

Maintenance Note

This registry is not a complaint ledger. It is a record of routing architecture as it functioned.

Entries should be added when:

  1. A custody gap is confirmed (FREF-0635-WWLV conditions met).
  2. The gap is structural, not incidental.
  3. A custody note was generated at receipt.

Do not add entries where testimony simply was not acted upon without confirming that the receiving layer held no routing authority. The distinction is custody architecture, not outcome disappointment.
