    WBA Continuity Gap — Cryosection Documentation Failure

    Incident Summary

    Subject 4-Bravo was dosed at T=0 and sectioned at 168h post-dose per protocol. The cryomacrotome operator ran a standard 40-micron serial section series. At section 38, a label printer buffer overflow assigned the next three labels to Study 2241-R rather than Study 2240-R.

    Sections 38–41 now exist in two study files:

    • Study 2240-R: sections documented as “not collected — equipment interruption”
    • Study 2241-R: sections documented as present, scanned, and within acceptable signal range

    Both studies were filed as complete. Both were GLP-compliant. The subject cannot be re-sectioned.

    Bricky’s Assessment

    Bricky reviewed the phosphor image plates for approximately six minutes, described the distribution as “pretty normal looking,” and submitted a one-line deviation report: “Label printer did a thing. Sections are fine. Filed under both. Recommend accepting both.”

    The Quality Assurance unit rejected this recommendation.
    Bricky acknowledged the rejection.
    Bricky’s original recommendation remains in the file.

    Current Status

    Regulatory submission for Study 2240-R is on hold pending resolution.
    Regulatory submission for Study 2241-R proceeded.
    Sections 39–41 are now cited in a published pharmacokinetic model.

    Both sets of documentation are complete. Neither set is authoritative. Nothing was resolved. The record now looks official.

    Identity has been forked; paper trails cannot substitute for missing raw traces. The form remained intact. The situation did not.

    Regulatory completeness masks epistemic failure. Meaning adjusted around the winning selector.

    A work breakdown structure guarantees that nothing works and everything is broken.

    We divide the tasks so the failure is distributed evenly.

    The baseline is a fiction we agreed upon to make the chart look nice.

    Assigning responsibility is the first step in ensuring nothing gets done.

    The schedule is a tombstone for time that was never really ours.

    And the Tribunal still hasn’t a clue.

    Which file’s the truth? Which is the hoax?
    The signal range indicates folks
    Were processed just right,
    Yet the labels took flight,
    And we’re split on the authoritative strokes.

    Bricky’s original note sits and waits:
    “Accept both.” That decision gates.
    QA rejected the call,
    But the note’s still in the hall,
    And the uncertainty permeates.

    The cryomacrotome’s cleaned and stored,
    The subject’s no longer aboard.
    We could guess, we could bet,
    But we can’t duplicate yet,
    And the label rot’s now on record.

    Bricky examined the phosphor plates,
    Said “Signal’s normal” then met his debate.
    Both files are fine,
    Both look divine,
    And he signed without dwelling on dates.

    Two-Two-Four-One is already out,

    The quality of the image is right,
