    Deviation Entered on Superseded Form — Meta-Deviation Cascade

    Incident Summary

    On April 19th, a study coordinator filed Deviation DEV-2026-0041 documenting a temperature excursion during sample storage.

    The form used was DEV-3A.
    DEV-3A was superseded on March 7th by DEV-4B.
    Use of a superseded form constitutes a deviation per SOP-089.

    A second deviation, DEV-2026-0042, was initiated to document the form error.
    DEV-2026-0042 was filed on DEV-3A.

    The QA reviewer identified this at 4:58 PM on a Friday.

    Bricky’s Closing Note

    “I have reviewed both deviations. They are both complete. The forms are wrong but the information is right. I have approved the information. I have noted the form issue. I have not filed a third deviation because I do not want to.
    The temperature excursion was 0.4°C for 11 minutes. The samples are fine.
    The forms are not fine. The forms are always not fine.
    Filed.”

    QA Disposition

    Both deviations remain open pending receipt of a CAPA (Corrective and Preventive Action) for the form version control failure.
    The CAPA form is Form CA-7.
    Form CA-7 was last revised in 2019.
    There is a Form CA-8.

    A temp excursion, point-four degree,

    Form Three was outdated,
    Form Four incarnated,
    On Six weeks prior decree.

    To use the old form meant a fault,
    So they filed a new one, no halt.
    But the new one was filed
    On Three-A, compiled,
    And the system began its assault.

    Bricky saw both deviations that day,
    At four-fifty-eight, Friday’s decay.
    “Both are complete,
    Information so neat,
    I don’t want to file a third way.”

    The deviation file references now

    The resolution requires a CAPA,
    The form is called CA-Seven’s saga.
    Last revised in twenty-nineteen,

    But the closure waits for the old paper.

    At five-oh-one, Bricky left,
    The deviation file bereft.
    Both still open,
    No closure spoken,
    And the temp excursion had less heft.

    DEV-0041 needs DEV-0042,
    Which needs CAPA—don’t you chafe.
    Each leads to the next,
    Each system is vexed,
    And the closure falls off the safe.

    “The forms are always not fine,”
    Bricky said at the deviation line.
    Which is not, he conceded,
    A solution we’ve needed,
    But it got everyone through Friday night.

    Both deviations remain open still,
    Awaiting a formal CAPA’s will.
    The temperature dip
    Was a minimal slip,
    But the form rot will take longer to fill.
